Will I achieve a “full recovery”?

Our goal is to maximize your recovery through patient-centered care. We believe that our wide range of services and our ability to treat our patients from the comfort and familiarity of their own home provides the best opportunity for you to achieve your highest functionality.

What does it mean to be campus-based?

We have dedicated staff present in our JSL senior living communities to provide permanent skilled nursing care and therapies to our patients.

Will you work with my doctor?

Yes. Your care starts with a prescription by your physician and we work at their direction. If you choose Supportive, we will be in direct contact with your doctor so that we can understand your medical history and provide the specific prescribed services.

How do I cover the costs?

Skilled home care is covered by Medicare and many insurances and, in most cases, does not require a co-payment.

How long will I need these services?

That is up to your physician and your progress. Our goal is to work with you, and your family so that you can manage your progress and recover as quickly as possible.

How can I get started?

Talk to your doctor or contact Supportive HHC to learn more about our services. Once we have a prescription from your physician, our goal is to open your case within 24-48 hours.
